  • Op-Ed Contributor - The Patients Doctors Don?t Know - NYTimes.com: Doctors are often unaware that 80-year-olds are not the same as 50-year-olds. Pneumonia in the latter causes fever, cough and difficulty breathing; in the former the same illness may have none of these symptoms. She may just seem ?not herself? -confused and unsteady, or unable to get out of bed. In a hospital she may receive a dose of antibiotic suitable for a 50 year old but is double what an 80-year-old should get leading to kidney failure, weakness and more confusion. In her confusion, she pulls the tube from her arm and the catheter from her bladder. Instead of re-evaluating whether the tubes are needed, her doctor then has the nurses to tie her arms to the bed which increases her agitation, keeps her bed-bound, and results in loss of muscle and bone mass. Eventually, she recovers from the pneumonia and her mind is clearer, so she?s discharged a frailer weaker woman who needs help with walking, bathing and daily chores.

  • Analysis from the Bottom Up | The Data Model That Nearly Killed Me: On February 17, 2009, President Barack Obama signed into law the economic stimulus package that appropriated about $20 billion for heath information technology (?Technology Gets a Piece of Stimulus?, New York Times, January 25, 2009. The American Recovery and Reinvestment Act of 2009, Subtitle A?Promotion of Health Information Technology, details the epically massive government program to digitize and network health information.) The law makes a job for yet another bureaucrat to oversee the vast program - is this change we can believe in? It defines rules for health information standards by designating a new standards board - everyone desires more data standards and standards groups. The law also explains how to test systems built with federal money but it does not explain how to measure semantic validity of information - garbage in garbage out! Good luck with all of that Mr. President.
